Appointment Request

Please review our office hours before requesting your appointment time. We will contact you within 24 hours to confirm any appointments. If you have not heard from us in that time, please call our office.

Be advised your appointment time is not finalized until we have confirmed with you.

Appointment Cancellation and No-Show Policy
Patients who miss an appointment without notice (No-Show) or cancel with less than 24 hours’ notice twice in succession will receive a formal notification from our staff. Following a third consecutive infraction, a $75 fee will be charged to the patient’s account. This fee must be paid in full before any future appointments can be scheduled. Cancellations made with 24 hours or more notice are exempt and will not count toward this policy.
